Office Policy


Paying For Your Care


In an effort to keep down the cost of your medical care by minimizing paperwork, our policy is payment upon completion of each visit. For your convenience we accept cash, check, MasterCard and Visa. There is a $30.00 fee that will be assessed on all returned checks.

Patients belonging to PPO's or HMO's will be asked to make the required co-payment as stipulated by the individual health plan. If there is a deductible amount due, it must be satisfied before a co-payment will be accepted as payment in full. We, at Kingstowne Family Practice, are continually working to increase the number of insurance companies with whom we participate. If you do not see your health plan company listed below, please give us a call for a status update since we are constantly updating our list of participating insurance companies.

We are providers for Medicare and TriCare/CHAMPUS, but do NOT accept Medicaid.


Missed & Late Appointments


Since our appointment schedule is limited, we again ask that you notify us at least 24 hours in advance if you cannot keep your appointment time. Failure to do so will result in a Missed Appointment charge. We routinely attempt to contact each of our patients the day before the appointment to remind them of the scheduled visit. If you will be arriving more than 5 or 10 minutes late, please let us know so that we may see the next patient in an effort to stay on schedule. You will be seen subsequent to that patient.


Telephone Calls


Patients are encouraged to call the office with questions they may have regarding medical problems or financial matters. Our office staff is trained to answer many of your questions. If they cannot, your question will be relayed to the physician who will return your call at his earliest opportunity. It is not usually possible for the doctor to leave patients to answer telephone calls.



Prescriptions & Renewals


We do not call in prescriptions without first seeing a patient for a specific medical problem. If you run out of long-term medications, it is usually because of a missed appointment. In such a case, please call our office to reschedule your appointment. A prescription for a few days' medication will be called in to your pharmacy until you can get back in to see your doctor. It will expedite matters if you have the pharmacy number available at the time of your call. You should contact the office at least 3 days before anticipating running out of medication. Same day refills cannot be guaranteed. Patients taking long-term medications must be seen by the doctor at least every 3 to 6 months to assure appropriate treatment.


Hospitalization


Dr. Leonidov is on staff at both Alexandria Hospital and Fairfax Hospital. Should hospitalization be required, the office staff will arrange for admission and appropriate follow-up. If referral to a specialist is necessary to assure optimal medical management, rest assured that Dr. Leonidov will be consulting with the specialist during your hospitalization.


Emergency Care


If you have an emergency, do not hesitate to contact our office and explain the urgency of your call. The doctor will be notified immediately.

After regular office hours, in case of emergency only, Dr. Leonidov is on 24-hour pager call through the answering service. For matters that can wait until normal office hours, we request that you leave a message for the next business day and not ask our answering service to page the doctor. In case of delay in making emergency contact with the doctor, or in the event of a life-threatening situation, proceed directly to the emergency room or call 911. The physician there will notify Dr. Leonidov of your situation. Maps to the local emergency rooms are available for the following hospitals:


New Patient Registration


If you have Adobe Acrobat Reader version 6 or higher installed on your PC, the registration forms for new patients may be obtained online by clicking on the PDF link below. If you do not have the program, you can download it for free here at Adobe’s Web Site.

PDF Format Registration Forms

Once the PDF file is opened, you may either print it out blank and manually complete it, or fill in out while on the screen before printing. If you use the screen-fillable option, please note that you will not be able to save your data to your PC. Either way, you MUST print and sign the forms, and bring them to the office. We do not take online submissions. Please arrive a few minutes prior to your scheduled appointment for processing of the forms, so that we may be able to stay on time.

To fill out the form on the screen, start by positioning your cursor in the first field (on the first line), then type the entry. Use the TAB button on your keyboard to move to the next field, or simply click in the middle of the next field to place the cursor there. To go back to the previous field, press SHIFT-TAB. Checkboxes are filled in by pressing the SPACEBAR on your keyboard to place an "X". Depressig the SPACEBAR again removes the "X". Alternatively, clicking directly on the box will insert an "X". To remove an "X", click again on the same box.

To print a PDF file from within your reader, we recommend you use the PDF's print button rather than your browser's print function.

If you have trouble opening the PDF file through your Internet browser, try RIGHT-clicking on the PDF icon and select "Save target as" from the resulting menu. Save the file to a floppy diskette, removable "thumbdrive" or your computer’s hard drive, and then open the file in Adobe Acrobat Reader.
